We believe that God wonderfully and immutably creates each person as male or female. These two distinct, complementary, and genetically distinct genders as determined at birth together reflect the image and nature of God (Genesis 1:26-27).
We believe that the term "marriage" has only one meaning: the uniting of one man and one woman in a single, exclusive union, as delineated in Scripture (Genesis 2:18-25). Sexual expression that pleases God is that which fulfills his intention at creation. The Seminary holds the position that such sexual expression is: (1) faithfulness in marriage between a man and a woman, or (2) chastity in singleness.
We believe that any form of sexual immorality (including adultery, fornication, homosexual behavior, bisexual conduct, transsexual conduct, bestiality, incest, and the use, sale and distribution of pornography) is sinful and offensive to God (Matthew 15:18-20; 1 Corinthians 6:9-10).
We believe that in order to preserve the function and integrity of Phoenix Seminary as an extension and manifestation of the Body of Christ, and to provide a biblical role model to the Phoenix Seminary family and the larger community, it is imperative that all persons employed by Phoenix Seminary in any capacity (faculty, administrative staff, student teaching assistants), or who serve as volunteers, agree to and abide by this Statement on Marriage, Gender, and Sexuality (Matthew 5:16; Philippians 2:14-16; 1 Thessalonians 5:22).
We believe that God offers redemption and restoration to all who confess and forsake their sin, seeking His mercy and forgiveness through Jesus Christ (Acts 3:19-21; Romans 10:9-10; 1 Corinthians 6:9-11). Hence, as noted above, we believe every person must be afforded compassion, love, kindness, respect and dignity. The Family Educational Rights and Privacy Act (FERPA) affords students certain rights with respect to their education records. These rights include:
The right to inspect and review the student’s education records within 45 days of the day the Seminary receives a request for access. A student should submit to the Registrar a written request that identifies the record(s) the student wishes to inspect. The Registrar will make arrangements for access and notify the student of the time when the records may be inspected. If the records are not maintained by the Registrar, the student will be advised of the correct official to whom the request should be addressed.
The right to request the amendment of student’s education records that the student believes are inaccurate, misleading or otherwise in violation of the student’s privacy rights under FERPA. A student who wishes to ask the Seminary to amend a record should write the Seminary official responsible for the record, clearly identify the part of the record the student wants changed and specify why it should be changed. If the Seminary decides not to amend the record as requested, the Seminary will notify the student in writing of the decision and the student’s right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing.
The right to provide written consent before the Seminary discloses personally identifiable information from the student’s education records, except to the extent that FERPA authorizes disclosure without consent. The Seminary discloses education records without a student’s prior written consent under the FERPA exception for disclosure to school officials with legitimate educational interests. A school official is a person employed by the Seminary in an administrative, supervisory, academic, research or support staff position (including mentors and coaches in the Seminary’s Mentoring Program); a person or company with whom the Seminary has contracted as its agent to provide a service instead of using Seminary employees or officials (such as an attorney, auditor or collection agent); a person serving on the Board of Directors; or a student serving on an official committee or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibilities for the Seminary. Upon request, the Seminary may also disclose education records without consent to officials of another school in which a student seeks or intends to enroll. Transcripts are provided to Ottawa University for students who are enrolled in the cooperative MAC program.

Seminary Use of Information
Phoenix Seminary collects and processes personal data ("Information") from individuals only as necessary in the exercise of the Seminary's legitimate interests, functions and responsibilities as a private, non-profit institution of higher education. For example, Phoenix Seminary also collects and processes Information from individuals who are applicants for employment positions. - Consent: We may disclose Information if we have your consent to do so.

The manner of destruction shall be appropriate to preserve and ensure the confidentiality of your information given the level of sensitivity, value and criticality to the Seminary.
Your Rights
You have the right to request access to, a copy of, rectification, restriction in the use of, or erasure of your information in accordance with all applicable laws. The erasure of your information shall be subject to applicable state and federal laws, and the applicable retention periods in the Seminary's Records Management policy. Use of the sites is subject to the terms and conditions set forth below in Phoenix Seminary’s Privacy Policy (“policy”).
Although Phoenix Seminary respects the privacy of this site’s users (or “members” or “visitors”), the Seminary collects information from its website members and visitors. The Privacy Policy governs users’ interaction with the site and users’ registration for and use of the site’s online services. THE INFORMATION PHOENIX SEMINARY GATHERS
Phoenix Seminary gathers two types of information about the site’s users:
- Non-personally identifiable information. As users access the site, Phoenix Seminary collects and aggregates information indicating, among other things, which pages were visited, the order in which they were visited, and which hyperlinks were employed. Collecting such information involves the logging of IP addresses, operating system, and browser software used by each visitor. Although such information is not personally identifiable, the Seminary can determine from the IP address a visitor’s Internet Service Provider and the geographic location of the user’s point of connectivity.
The non-personally identifiable information collected helps in diagnosing any server problems, administering the site, identifying the most popular areas of the site, and determining the effectiveness of promotional activities.
For the same purposes, the Seminary uses “cookies” – that is, small text files placed on your computer’s hard drive – to help determine the type of content and sites visited, the length of time spent on a particular site, and the usage of services offered via the site. - Personally identifiable information. HOW THE SEMINARY USES/DOES NOT USE GATHERED INFORMATION
Please take some time to become familiar with the different ways Phoenix Seminary uses the information gathered. Keep in mind that, while the Seminary encourages all third parties involved to adhere to this site’s Privacy Policy and to otherwise handle personal information in a responsible manner, Phoenix Seminary cannot and does not assume any responsibility for any actions or omissions of third parties, including the manner in which they use information received either from the Seminary or independently. Nevertheless, in the event users encounter any third party associated with, or who claims association with Phoenix Seminary, who is improperly collecting or using personal information, please contact webcomm@ps.edu who will forward the message to the third party.
- Use of non-personally identifiable information. The Seminary uses non-personally identifiable information in aggregate form to build higher quality, more useful online services by performing statistical analyses of the collective characteristics and behavior of the site visitors, and by measuring demographics and interests regarding specific areas of the site.
The site may contain links to other Internet Web site, including co-branded or other affiliated sites that may or may not be owned or operated by Phoenix Seminary. - Use of personally identifiable information. Phoenix Seminary limits its own e-mail contact and solicitations to those users who have not “opted out” of receiving such communications. The solicitations are limited in that they, among other things, will clearly indicate the originator of the e-mail and provide the user with a method of “opting out” of receiving future communications of a similar nature.
Finally, Phoenix Seminary may use individual members’ data to “pre-populate” forms, which are displayed for the purpose of collecting individual data by the Seminary. In no case does pre-populating a form transfer any data to any third party. Only if the user voluntarily requests that such data be transferred will any transfer take place – for example, if/when a user clicks a “submit form” button or other button.
Phoenix Seminary reserves the right to release any and all information contained within access logs concerning any visitor when that visitor is in violation of the Seminary’s published guidelines, or partakes (or is reasonably suspected of partaking) in any illegal activity, even without a subpoena, warrant, or other court order, and to release such information in response to discovery requests, or in response to any circumstance in which the Seminary, in its sole discretion, deems an emergency. Phoenix Seminary cooperates with law enforcement agencies in identifying those who may be using its servers or services for illegal activities. Phoenix Seminary also reserves the right to report any suspected illegal activity to law enforcement for investigation or prosecution.
Phoenix Seminary will post the relevant privacy information in the official rules and/or registration area for any promotions it sponsors. That privacy information, to the extent it conflicts with this Privacy Policy, will govern that particular promotion.
SECURITY
All information gathered on the site is stored within a controlled database accessible only to the Seminary to protect the loss, misuse, or alteration of the information under its control. However, as effective as any security measure implemented by the Seminary may be, no security system is impenetrable. Miscellaneous
This agreement is entered into in the state of California and shall be governed by and construed in accordance with the laws of the state of California, exclusive of its choice of law rules. Each party to this agreement hereby submits to the exclusive jurisdiction of the state and federal courts in the state of California, and waives any jurisdictional, venue, or inconvenient forum objections to such courts. In any action to enforce this agreement, the prevailing party will be entitled to costs and attorneys' fees. In the event that any of the provisions of this agreement shall be held by a court or other tribunal or competent jurisdiction to be unenforceable, such provisions shall be limited or eliminated to the minimum extent necessary so that this agreement shall otherwise remain in full force and effect and enforceable.
This agreement constitutes the entire agreement between the parties hereto pertaining to the subject matter herein, and any and all prior or contemporaneous written or oral agreements existing between the parties hereto are expressly canceled. No waiver by either party of any breach or default hereunder shall be deemed to be a waiver of any preceding or subsequent breach or default. Any heading, caption, or paragraph title contained in this agreement is inserted only as a matter of convenience and in no way defines or explains any paragraph or provision herein.
Phoenix may at any time change the site, including eliminating or discontinuing any content or feature of the site, restricting its availability or limiting the amount of use permitted. Such changes, modifications, additions, or deletions shall be effective immediately upon notice thereof, which may be given by any means including, but not limited to, posting on the site, or by electronic mail, or by conventional mail. Use of the site after such notice shall be deemed to constitute user's acceptance of such changes, modifications, additions, or deletions.

By proceeding with registration, you are consenting to conduct transactions and enter into agreements electronically, with no paper confirmation of said transactions/agreements to be provided. These transactions/agreements may occur at any time during your academic career and may include but are not limited to any of the below stated items. Phoenix Seminary may provide communications and engage in transactions with you in paper form at any time, despite this consent. This includes receiving all communications via the institution’s primary method of contact, which includes but is not limited to personal email account, via the student portal, PS.com or by phone/text to any active phone number provided by me in my student record. Communications may include the following:
- Student Accounts documents that may include but are not limited to billing statements, statement changes, payment plan changes, payment due notifications, late payment notices, account holds, collections, legislative notice requirements, and tax document availability.
- Registration Documents that include but are not limited to FERPA Release Requests, Graduation Applications, ordering Official Transcripts, release of information to third parties, and Student Verification Request Forms.
- Admission Documents that include but are not limited to Applications for Admission, Admissions Offer Acceptance.
- Financial Aid Documents that include but are not limited to Accepting/Declining Financial Aid Awards, Release, or Return Funds including Title IV funds.
- Miscellaneous Documents that include but are not limited to receipt of consumer information about the Seminary that may be required by law or regulation such as any disclosure, consent, waiver, release, or notice.
- This consent also includes consent to receive your 1098T statement electronically in the student portal, where it will be available annually from January 31 for a minimum of three years from issuance. If you want to retain a paper copy of any record provided electronically you should print a copy from your student portal account. Any post-consent request for the statement in paper form will be considered a withdrawal of consent.
I specifically consent to receive my year-end 1098T tax form electronically via my student portal unless I have taken the appropriate measures to opt-out of electronic delivery, in which case a paper copy will be made available. To opt out and request a paper copy, I must email a notice to: finance@ps.edu
Written notice must include your specific request to opt-out of the electronic delivery option for your 1098T document and verification of your current address on file with the institution. If your address is incorrect, you must update your address information via student your portal prior to submitting this request. My request must be received by Phoenix Seminary no later than October 31st of the tax year for which I wish to elect paper delivery. If I do not fulfill the opt-out requirements by the stated deadline, I will be able to view and print a copy of my 1098T by accessing my student portal. Although the duration of this agreement is indefinite, I may withdraw my consent by following the instructions above. Confirmation of my consent withdrawal will be sent to my personal email address.
In order to conduct business electronically with Phoenix Seminary, you must use a computer with a supported operating system and internet browser, sufficient electronic storage capacity on the computer's hard drive or other data storage unit, a printer that is capable of printing from the browser, and email software.
